History

54-year-old woman with 4 days of right-sided pleuritic chest pain and exertional breathlessness, 3 weeks after knee arthroscopy and a long-haul flight. Heart rate 104/min, SpO₂ 93% on air, blood pressure 128/78 mmHg. Wells score: PE likely. D-dimer 2,900 µg/L. Chest radiograph normal. Chronic kidney disease, eGFR 38 mL/min/1.73 m². Low-molecular-weight heparin started. Referred for V/Q SPECT: pulmonary embolism?
