Two years of forgetfulness in a 71-year-old teacher

History

71-year-old retired teacher with a 2-year history of forgetting recent events and repeating questions. Independent in personal care but has handed the household finances to her husband. MMSE 24/30. MRI brain: mild bilateral hippocampal atrophy, two lobar microhaemorrhages, no superficial siderosis. Not on anticoagulants. Referred for FDG PET: is the pattern consistent with Alzheimer's disease? The memory team is considering amyloid-targeting therapy.

Compared with a normal reference study, metabolism is reduced in the temporoparietal cortex on both sides, the posterior cingulate and the mesial temporal lobes; the frontal and sensorimotor cortex, basal ganglia and cerebellum are preserved. How would you describe the metabolic pattern?
